We're developing a new questionnaire for people who report UFO sightings or other anomalies. We want to know how they are different from the general population - what they have experienced and what they believe.

We would like your help, if you have a 5 minutes you can spare.

The questionnaire is anonymous. Enter case number 18-999, and answer all of the questions you feel comfortable answering, as accurately as possible. You can then comment on the questions in Section 5.

Not sure how to be specific but I like the ratings 1-5 starting on page 4. I'd like to see more of those but honestly can't think of what to ask at the moment, just liked the format.
Couldn't help but chuckle at a few of the questions but understand why they are there - when it's raining you put on a raincoat.
